You can do this, actually.

You can create your own custom actions at Prefs > Mapping > QuickTime 
Player > "Select 'Custom Actions..' from the popup". Actions defined 
there are can not only be mapped to buttons, but they also become 
available as menu items.

Nope, I have no menu items available. I can only change the key mappings. The menu items do not appear In Prefs > Menu > Quicktime Player.

I even looked through most of the bottom three trees. Custom key commands are only accessible via Prefs > Mapping.

If I'm missing a step let me know. Clicking the Plus sign under the "menu layout" pane does nothing. As I understand, Items are added from the item library pane.
