Hi Felix

Is there a way I can use QT to play a movie on fullscreen as a default without having to press the middle remote button

I'm sorry, but right now, there's no built-in way to do this. I've 
added it to the ToDo list, though.

The long workaround hack method is to create an applescript app to open the quicktime, then go to full screen.

There are complications with adding the extension type to the applescripts info.plist, but it does work (I have exactly this working, but as part of a larger more complicated setup - which I might detail in another post).
